By completing this form I agree that I am legally entitled to represent the person/s photographed and grant full copyright and all rights to The Performer's Lens (Millie Jameson) for cosplay and modelling photographs and accept The Performer's Lens general terms and conditions.

I agree that The Performer's Lens (Millie Jameson) must approve all participants in a photoshoot and each approved participant must complete this model release form. 

I consent to the use of the photograph/s, reproduction, or any manipulated image/s in digital publishing, advertising, trade or for any other legal purpose without reservation.
