By signing this form I (The Client) agree to participate in a photoshoot with The Performer's Lens (Millie Jameson). I accept and agree to the Terms & Conditions made available on theperformerslens.co.nz website at this time. 

Here is a brief summary of key points: 

• Agreement for the photoshoot (and any later variations or changes) must be in writing
• Full payment is required on the day of photoshoot or, by agreement, within 7 days
• 24 hours notice must be given for postponing or cancelling a photoshoot (fees may apply)
• The Performer's Lens reserves the right to postpone or cancel a photoshoot due to adverse weather
• The Client will following reasonable health and safety instructions provided by The Performer's Lens
• The Performer's Lens retains Copyright for the Photographic Work and grants a non-exclusive license to the Client. 
• The Client can not alter, manipulate, or partially use images without Photographer’s prior written permission except for very minor edits such as changes to saturation, contrast etc.
